About the role: 

We are looking for experienced nurses and patient champions to work in the Community Health Service within a centralised Hospital in the Home team. In this role you will provide acute community based nursing for patients to support admission avoidance and early support discharge pathways. 

The Registered Nurse role will work as a member of an interdisciplinary Hospital in the Home team which includes SMO, Nurse Practitioners, CNS, District Nurses, Allied Health and HCA staff working across the Counties Manukau Health district. 

The Hospital in the Home team will provide a mixture of remote monitoring and face to face patient assessments/intervention with rostered shifts between 8am-6pm, Monday - Sunday. You will provide support and intervention adapting a multi-disciplinary approach to patients and working alongside their whanau in the community.

The Registered Nurse will provide evidence-based care to optimise patient outcomes and ensure the services delivers high quality care. The successful applicant will be required to demonstrate nursing competence and knowledge sharing, guidance and support within the team and will actively establish relationships with other stakeholders. 

You will be required to initiate and develop innovative approaches to problem solving while utilising skills and input from other disciplines and team members to co-ordinate patient care.

You will be empowered to make decisions or recommendations relating to patient management, while demonstrating a high level of communication and professionalism. 

 

Essential requirements: 

  • Registered with the NZ Nursing Council
  • Functioning at an RN Proficient level of PDRP
  • A minimum of 3 years recent clinical experience in acute care
  • Knowledge and experience in long-term conditions care coordination 
  • Full Drivers Licence
