We are seeking a Community Liaison Clinical Nurse Specialist (CNS) who is an energetic, forward-thinking clinician, who thrives in a dynamic fast paced environment, and is passionate about improving patient flow and transitions of care. The Community Liaison CNS role is a senior nursing position focused on optimising acute patient flow and ensuring safe and timely transitions of care from hospital to the community. You will act as a clinical expert, liaison and navigator for patients with complex discharge needs working across acute, medical, surgical and speciality wards.
The Community Liaison CNS role is supported by the CNS Team Lead and the Clinical Nurse Director, Specialist Community Health Service. A detailed position description is available on request.
Core responsibilities:
- Proactive identification of inpatients suitable for community-based care (such as Hospital in the Home (HiTH), remote patient monitoring and Rehab at home)
- Lead and coordinate complex discharge planning
- Promote and educate clinicians on community pathways and resources
- Collaborate with acute care, ED, medicine, surgery and speciality wards
- Provide advanced clinical assessment and decision making
- Role model expert practice and evidence-based care
- Support whaanau-centered, equitable and culturally safe care
- Communicate effectively and negotiate with an ability to influence across multidisciplinary teams
Essential Criteria:
- Registered Nurse with a current Nursing Council of New Zealand Annual Practising Certificate
- Post graduate Diploma in Nursing or working towards.
- Minimum of 5 years post registration experience, including acute nursing and hospital experience.
- A strong commitment to holistic, person- and whaanau-centred care.
- A Proficient, Expert or Senior Nurse level PDRP.
- Advanced clinical assessment and critical thinking skills.
- Effective problem solving, priority setting and planning skills.
- A commitment to participating in continuous quality improvement.
- Solution focussed, team player with excellent communication skills.
- Innovative and creative clinical practice that is validated with evidence based best practice.
