About the role: 

If you are an Allied Health or nursing professional and have an interest in working with older people and general rehabilitation, this may be the role for you.

We have an opportunity for a registered health professional to join our Needs assessor and Service Co-ordinator (NASC) team. The NASC team, work with patients admitted to our Health of Older People or Neuro rehab wards.

This role would suit a Registered Health Professional who is looking for a change while continuing to use your clinical assessment skills and training. 

This important role forms part of the multidisciplinary team and primarily involves assessing the needs of Older People over 65, and at times people under 65. You will be working with patients and family/whanau carrying out specialist assessments using the interRAI assessment tool to develop personalised support plans to ensure a safe and effective discharge from hospital to their own homes or other community living settings.

This role you will be proactive in identifying patients that require needs assessment, working closely with the wider team. Your communication style will be clear with excellent written and verbal communication and able to work effectively in a team environment. 

In this role you will have the support of the interdisciplinary team and supervision provided by senior colleagues, and will receive a comprehensive orientation.

 

Essential:

  • NZ Registered Health Professional with a current Annual Practising Certificate and Professional body registration such as a Registered Nurse, Occupational Therapist, Physiotherapist, or Social Worker
  • Comprehensive clinical assessment skills
  • Excellent verbal and written communication and computer skills.
  • 2 years’ experience working in a multi-professional, inter-disciplinary health or disability related environment
  • Be able to meet the demands of a fast-paced health environment.
  • Current full clean drivers licence

 

Desired:

  • interRAI assessment trained
  • Working with the elderly in a hospital or community setting.
