About the Role

Make a meaningful difference in the lives of people navigating mental health and addiction challenges. This role sits within the Needs Assessment and Service Coordination team in the Adult Community Mental Health and Addictions Service in Tauranga.

As a Registered Mental Health Professional, you will work alongside Tāngata Whaiora and their whānau to develop personalised recovery plans and coordinate the community resources needed to support independence and wellbeing. The role involves comprehensive needs assessments, collaborative care planning, and coordination of community support services. Strong partnerships with specialist clinical teams, support providers, housing services, and other community agencies are central to achieving positive outcomes.

This position also offers opportunities to contribute to service development initiatives, including supported housing approaches and the implementation of shared care pathways across clinical and community services.

About the Team/Service/Location

Thee Needs Assessment and Service Coordination team forms part of the Adult Community Mental Health and Addictions Service in Tauranga. This small, dedicated team works closely with clinical services, community providers, and government agencies to ensure people receive the right support at the right time. The team plays a key role in Bay of Plenty’s integrated care approach, helping coordinate services that enable Tāngata Whaiora to live well in their communities while promoting recovery, independence, and meaningful engagement with support networks.

About You

You will be an experienced Mental Health Professional who values collaborative practice and recover focused care. Calm, adaptable, and confident working autonomously, you bring strong assessment skills and the ability to build trusted relationships with Tāngata Whaiora, whānau, and a wide range of community partners. A partnership approach sits at the heart of your practice, and you take pride in supporting people to achieve their goals and aspirations.

What you will bring:

  • Registration with the relevant professional body as a Registered Nurse, Occupational Therapist, or Social Worker with a current Annual Practicing Certificate
  • Experience working within mental health and addiction services
  • Strong assessment, rehabilitation planning, and care coordination skills
  • Ability to work independently while contributing to a collaborative multidisciplinary team
  • Experience building effective relationships with community providers and government agencies
  • Commitment to recovery focused, culturally responsive practice and improving outcomes for Tāngata Whaiora and their whānau
