About this role

The Clinical Nurse Specialist (CNS) Vascular Access and Outpatient Antimicrobial Therapy (OPAT) is a unique role that works across the care continuum. You will be responsible for Case management of Outpatients on intravenous and / or oral antimicrobial therapies and Vascular Access management, education, training, and support (Inpatient and community) within Te Whatu Ora CM Health.

 

Key Responsibilities:

  • Collaborate with the multidisciplinary team (MDT) to deliver a high-quality, patient-centred Outpatient Parenteral Antimicrobial Therapy (OPAT) service.
  • Assess patients for suitability for home-based antimicrobial therapy and support patients, whaanau, and carers to manage treatment safely and confidently at home.
  • Coordinate patient discharge into the OPAT service and provide ongoing case management throughout treatment.
  • Deliver an on-call telephone support service (0800–2200, seven days a week), providing clinical advice, vascular access support, and management of treatment-related complications.
  • Develop and deliver evidence-based education and competency programmes in vascular access device (VAD) management for clinical staff.
  • Provide clinical leadership and specialist expertise in vascular access management and the care of patients receiving long-term intravenous or oral antimicrobial therapy.
  • Lead quality improvement initiatives and promote best-practice standards in Vascular Access management and patients on long term antimicrobials.
  • Contribute to service development and patient flow initiatives that support safe, timely discharge, best practice and continuity of care.

 

Essential:

  • Registered Nurse with New Zealand Nursing Council
  • At least 3-5 years post registration experience in a relevant clinical environment
  • Relevant Postgraduate diploma qualification (or working towards)
  • Current Annual Practicing Certificate
