About the role: 

We are looking for experienced nurses and patient champions to work in the Community Health Service within a centralised Hospital in the Home team. This Clinical Nurse Specialist (CNS) role provides an acute community-based nursing for patients to support admission avoidance and early discharge pathways. This role will be a member of an interdisciplinary Hospital in the Home (HiTH) team which includes Senior Medical Officers, Nurse Practitioners, Clinical Nurse Specialists, Registered Nurses, Maori team, Allied Health and Health Care Assistants staff working across the Counties Manukau Health district.

The HiTH CNS role is multi-functional and will rotate through the following duties across the Counites Manukau district and the wider Norther Region:

  • Home visiting, delivering face to face care
  • Virtual nursing, utilising digital tools to provide remote patient care
  • Triaging for Counties Manukau HiTH
  • Day-time coordination for Counites Manukau HiTH
  • Overnight Coordination for the Regional Virtual Hub, supporting the virtual team to provide remote patient monitoring and escalation support to patient from across the Region HiTH network

The CNS will:

  • Utilised advance nursing assessment skills and knowledge to contribute to the delivery of safe and effective patient and whaanau centred care that positively impacts health outcomes
  • support adapting the Te Whare Tapa Wha framework to patients and working alongside their whanau in the community.
  • provide expert, evidence-based care to optimise patient outcomes and ensure the services delivers high quality care. 
  • be effective role models demonstrating high levels of nursing competence and knowledge sharing, 
  • provide leadership, guidance and support within the team

Initiate and innovative approaches to problem solving will be required while utilising skills and input from other disciplines and team members to co-ordinate patient care. The CNS is empowered to make decisions or recommendations relating to patient management, while demonstrating a high level of communication and professionalism. 

 

Essential requirements:

  • Registered Nurse with a current NCNZ annual practicing certificate
  • Completed post graduate diploma including an advance clinical assessment paper.
  • Post graduate studies in pharmacology will be an advantage.
  • A minimum of 5 years recent clinical experience, preferably a combination of acute and community care.
  • Knowledge and experience in long-term conditions care coordination and case management
  • Full Drivers Licence
