About the Role
Waitemata District has launched a new service supporting patients through their inpatient journey and in the community - Supported Transition and Restorative Team (START). Our aim is to provide a service for patients who can benefit from a short period of rehabilitation in their own home, to help restore their functional ability.
We are seeking an experienced Registered Nurse to join the new transdisciplinary team supporting patients through their hospital to community journey. The Registered Nurse role will be integral in identifying and providing close management of patients through their rehabilitation pathways through co-ordinated assessment and clinical care. This will ensure successful patient outcomes, hospital flow and timely and effective utilisation of supports available.

About the Team/Service/ Location
Waitematā START focuses on providing inpatient and community support for patients to achieve their short-term functional goals and increase their independence with activities of everyday living.
Patients are supported by the transdisciplinary team of Clinicians from Nursing and Allied Health disciplines supported by a strong team of Restorative Assistants.
About you
To be considered we will need the following:
- New Zealand Registered Nurse (RN level 4), with a current Annual Practising Certificate and experience with patients with complex health needs
- Contemporary clinical professional experience across inpatient and community settings ideally in Orthopaedic, Health of Older People, Gerontology, Rehabilitation and / or Restorative care
- Experience with monitoring patient pathways and data management
