About the Role
We are seeking an enthusiastic, passionate and skilled registered health professional to join our Health of Older People team at Disability Support Link.
This will be a varied and exciting role that will work alongside clients, families, local communities and inpatient settings so that the process of a needs assessment and service coordination reflects both community strengths and client needs.
You will be working in partnership with older people who have disability needs, and younger people with long-term health conditions, to achieve optimum health and well-being through appropriate assessment, planning, intervention, evaluation and education.
Key responsibilities:
- Skilled clinical assessment
- Coordination of support services
- Effective, safe and timely communication with clients/whanau
- Multidisciplinary teamwork
- Knowledge of local community options

About the Service
Disability Support Link covers a wide geographical area.
We work with Home and Community Service Providers and Age Related Residential Care facilities to support 10000 clients across our Motu.
We complete interRAI assessments and coordinate long term care in the community and aged residential care.
We are engaged in change as Health of Older People Needs Assessment Service Coordination agencies across Aotearoa work towards national standardisation of processes.

About you
Skills and experience you’ll need to be considered for the role:
- Commitment to self-development and the development of others
- A current NZ annual practicing certificate
- Full New Zealand drivers licence
- Strong teamwork and a collaborative approach
- InterRAI Home Care competency is an advantage but not necessary as full training will be offered
