Monday to Friday, 8.00am – 4.30pm

An exciting opportunity has become available for an experienced Registered Nurse, Social Worker or Occupational Therapist to join the Te Toki Haumanu multidisciplinary team within the Rotorua Adult Community Mental Health & Addiction Service. This unique role combines the delivery of Closer to Home intensive community-based mental health care with Adult Mental Health Triage, supporting tangata whaiora and their whānau from their first point of contact through assessment, early intervention, intensive treatment and transition to ongoing care.

About the Role

Working as part of the Te Toki Haumanu team, you will provide high quality, recovery-focused mental health assessment, triage and intensive community intervention for adults experiencing moderate to severe mental health and addiction needs. This role aims to improve access to specialist services through early intervention, reduce inequalities and clinical risk, and minimise the need for admission to Whare Whakaue Ward or other specialist inpatient services.

Your responsibilities will include:

• Providing comprehensive mental health assessments, triage, risk assessment and psychosocial interventions.
• Delivering intensive, evidence-based mental health care in the community to support people to remain safely in their own environment wherever possible.
• Facilitating timely access to Adult Community Mental Health & Addiction Services and ensuring a safe and coordinated patient journey across the care continuum.
• Supporting tangata whaiora and their whānau from first contact through early acute intervention and safe transition to ongoing services.
• Developing collaborative recovery plans that promote wellbeing, independence and ownership of recovery.
• Working closely with multidisciplinary colleagues and building strong partnerships with Primary Health, Specialist Services, NGOs, iwi providers and other community organization's.
• Ensuring referrals and transitions of care occur in partnership with tangata whaiora and whānau wherever appropriate.
• Contributing to service development as Te Toki Haumanu continues to strengthen equitable access to mental health services.
• Delivering care aligned with Te Tiriti o Waitangi, Te Ara Tauwhirotanga and Te Ao Māori models of care.

About You

You are an experienced mental health clinician who enjoys working autonomously while being part of a supportive multidisciplinary team. You are committed to delivering person and whānau-centred care and improving equity of health outcomes for our community.

You will have:

• Registration as a Registered Nurse or other suitably qualified Mental Health Clinician with a current Annual Practicing Certificate and registration with the appropriate professional body.
• A minimum of three years' experience within a secondary specialist mental health service.
• Strong assessment, triage, risk assessment and psychosocial intervention skills.
• Proven ability to work independently and collaboratively across multidisciplinary and interagency settings.
• Excellent communication and relationship-building skills.
• Postgraduate qualifications in Mental Health and Addiction (or working towards these) would be an advantage.
• Competency in Te Ao Māori, tikanga and te reo Māori, or a commitment to developing these capabilities.
• Experience applying Te Tiriti o Waitangi principles, Mātauranga Māori and Kaupapa Māori approaches within healthcare practice.
