Waitematā District is developing an exciting new interdisciplinary Service to support patients restore functional abilities in the community - the Supported Transition and Restorative Team (START)

About the Role

Health New Zealand | Te Whatu Ora is firmly grounded in the principles of Te Tiriti o Waitangi and is committed to building a health system that serves all New Zealanders.

START patients are supported by a specialised Interdisciplinary team (IDT). Services span from the inpatient environment through to support at home in the community via in person visits and Telehealth.

The START physiotherapist, together with other clinicians will case manage patient support in the community through setting, progressing and assessing achievement of patient and whānau centred goals and coordinating restorative care activity across the IDT, including the utilisation of the Allied Health Assistants team.

START clinicians will provide clinical oversight and supervision to all Allied Health Assistants within the service, ensuring support is provided for training and competency development.

The Physiotherapist, Key Worker will enhance health outcomes for patients and whānau by providing clinical and highly developed knowledge in clinical practice in order to provide best care.

About the Team/Service/ Location

Sitting in the Specialty Medicine and Health of Older People (SMHOP) Division the START team will support patients across the Waitematā District, based initially in two community hubs of North Shore Hospital and Waitākere Hospital delivering support services in the community.

About you

To be considered for this role you must meet the following criteria:

  • Be a role model for patient and whānau centred care
  • The ability to apply developed knowledge, skills and clinical reasoning when providing evidence based practice
  • Excellent Coordination skills, be able to facilitate and model high quality professional and interdisciplinary practice.
  • Provide guidance to other staff, based on contemporary best practice.
  • Provide clinical teaching to patients, families and whānau, students, staff and other health professionals.
  • Demonstrate leadership to support service development, as agreed with the START Community Lead:
    • Clinical Supervision of staff
    • Training and education
    • Supervision of students
    • Coaching of others
    • Professional development activity, e.g. development of best practice guidelines and models
    • Day to day clinical caseload prioritisation and staff mix management
    • Other specialist clinical resources as required by the team

Clinical cover is provided across both hospitals and on statutory holiday and as part of a weekend roster. Services are delivered in accordance with Health NZ I Te Whatu Ora Waitematā Strategic Plans.
