Community Team Lead, Community Servies, Taranaki Base Hospital, Permanent Fulltime

About the Role

We’re looking for someone to provide leadership across our community health services, including Needs Assessment and Service Coordination (NASC), Home Support, and the Community Health Integration Centre (CHIC). The role also supports our small, specialised nursing workforce. You’ll work closely with teams and partners to deliver integrated, person-centred care, guide service development, and manage resources effectively to ensure quality and sustainability.

About the Team/Service/ Location

NASC, Home Support, CHIC, and our nursing team work within the wider Community Services team. The NASC, Home Support and CHIC are a dedicated group that provides service coordination for people requiring services or supports living in the community. The nursing team provides clinical support within the wider Community Services framework.

About you

  • A relevant graduate qualification/postgraduate in Nursing or Allied Health
  • Current Annual Practicing Certificate or Registration
  • Sound knowledge of the health of older people
  • Holds a full New Zealand Drivers Licence
  • 5 years leadership experience
  • Knowledge of community-based services
  • Ideally experience with service coordination
