About the Role
The Bay of Plenty Care Coordination Centre serves as a centralised access point for patients needing short‑term health services across the region. We are looking for an experienced Registered Health Professional/Registered Nurse to join us as a Community Care Coordinator, based at the Te Whare Whakamana building on 17th Avenue, across from the Historic Village.
This position involves compassionate client interactions and collaborative work with healthcare professionals, using phone and technology‑based communication to guide clients toward the most appropriate services.
This is a permanent, full‑time role (1.0 FTE, 40 hours per week) and requires flexibility to cover rotating weekend shifts.

About You
Success in this role requires a solution‑focused mindset, a collaborative approach, and the ability to identify opportunities to enhance service integration. Key responsibilities include screening requests, determining appropriate responses, and fostering strong relationships with healthcare providers across community and hospital settings.
What you will bring:
- Registered health professional with current Annual Practicing Certificate
- A relevant graduate qualification (E.g. Registered Nurse)
- Strong knowledge of patient management systems and medical terminology
- Experience working in a health or disability environment
- Experience in assessing and coordinating patient care
- Experience in client/whānau engagement and support
- Ability to work without supervision in an environment of change and variability
