About the Role
As a Clinical Pharmacist Facilitator, you will work directly within primary care teams to support safe, effective and evidence‑based use of medicines across Te Matau a Māui, Hawke’s Bay. Your role centres on improving medication outcomes for patients by providing expert clinical input, independent medicines reviews, medicines reconciliation, and timely advice to general practice, nurses, prescribers and community providers.
You will help implement best‑practice guidelines, support quality improvement initiatives, produce clinical education, and interpret prescribing and diagnostic data to influence safe and effective prescribing. This is a dynamic and relationship‑driven role where you will contribute to better health outcomes and enhance the capability of primary care teams through partnership, coaching and clinical expertise.

About the Team/Service/ Location
You’ll be part of the Clinical Pharmacist Facilitation team within the Pharmacy Department, working collaboratively across primary care settings throughout Hawke’s Bay. The service works closely with general practice teams, Māori and Pacific providers, community pharmacies, age‑related residential care, Health Hawke’s Bay (PHO), and broader Te Whatu Ora services.
This is a supportive, knowledgeable and highly collaborative team that values evidence‑based practice, quality improvement and strong partnerships. You’ll be well supported by a team leader, pharmacist colleagues and a wide network of clinicians, with opportunities to contribute to region‑wide initiatives and innovative models of medicines management.

About you
You’ll be an experienced and confident pharmacist who enjoys working autonomously and collaboratively in a primary care environment. To succeed in this role, you will bring:
- Current registration and APC with the Pharmacy Council of New Zealand.
- A relevant postgraduate clinical pharmacy qualification — or be working towards one — or equivalent clinical experience.
- Strong clinical practice capability, including:
- Medicines therapy assessment or related medicine review experience
- Evidence‑based advisory skills
- Experience promoting safe and high‑quality medicines use
- Critical appraisal and medicine information provision
- Quality improvement experience
- Excellent relationship‑building skills and the ability to work alongside GPs, nurses, pharmacists and community health providers.
- Confidence working independently, using clinical judgement and adapting to evolving primary care needs.
- A full NZ driver’s licence.
Experience with general practice systems, prescribing data tools, or project management is beneficial.
