About the Role

We need a Nursing or Allied Health Clinician to join our Adult Community and Long-Term Conditions Service based at Te Toka Tumai - Auckland. As service co-ordinators, our team is committed to delivering support services to people whose health needs need to be met at home in a locality-based framework. Our community team provides assessment and service coordination to adults who have long term disabilities or chronic long-term health conditions.

About you

We welcome applications from Nursing or Allied Health Clinicians with practice experience and who meet the following criteria:

  • NZ registered health professional with current annual practising certificate
  • Clean full NZ drivers licence
  • Open to new challenges and experiences
  • Experience working in the community setting is desirable
  • Commitment to the development and implementation of quality improvement initiatives
