Needs Assessor - Registered Nurse/Allied Health Professional

About the Role
We are seeking a registered health professional with a current practising certificate to join our Older Persons Needs Assessment Service Coordination (NASC) team. In this role, you will work closely with individuals, their whānau and support networks to understand needs, complete comprehensive assessments, and coordinate services that enable people to live well in their homes and communities.
You’ll play an important part in supporting older people and those living with long-term health conditions across Hawke’s Bay, ensuring equitable access to the right care, at the right time.

About you
As a Needs Assessor, you will bring the clinical experience, professionalism, and relationship‑centred approach needed to work effectively with older people, those with long‑term health conditions, and their whānau. You will also demonstrate a commitment to equity and the principles of Te Tiriti o Waitangi.
You will have:
- A current practising certificate and at least two years’ experience in your relevant profession (RN or Allied Health).
- Strong assessment, clinical reasoning, and documentation skills, ideally including experience with InterRAI.
- Excellent communication skills and the ability to engage effectively with a wide range of people, including whānau and community providers.
- The ability to work both autonomously and collaboratively within a multidisciplinary team in a busy environment.
- Strong organisational skills, with the ability to prioritise, manage your workload, and maintain personal wellbeing in a demanding role.
