About the Role

Join a dynamic, social work team committed to whānau-centred care and health equity in the heart of Te Tairāwhiti. As the ED Social Work- Care Transition Lead is situated in the Emergency department, you’ll play a vital role supporting patients and whānau through a range of psychosocial challenges in emergency and community settings. There is a focus of admission avoidance and ensuring patients are supported in a timely manner .Each day brings variety – from crisis intervention and discharge planning to connecting clients with local iwi and community services.

About the Team/Service/ Location

We have a diverse team which extends across the hospital with inpatient wards, ICU, Planet Sunshine, Emergency, Tui te Ora (Long-term conditions), Oncology. In addition to this we have Kaiatawhai/ Social workers and Family Violence Intervention Coordinator. You'll work alongside passionate health professionals who value collaboration, cultural safety, and the principles of Te Tiriti o Waitangi. About you

As a social worker you will require you to have:

  • Experienced and skilled in conducting psychosocial assessments and developing responsive intervention plans
  • Strong verbal and written communication to engage with clients, whanau and multidisciplinary teams
  • Passionate about reducing health disparities and improving outcomes for vulnerable populations
  • Ability to work respectfully and effectively with Maori, Pacific peoples, and diverse communities applying Te Tiriti o Waitangi principals
  • Recognised Social Work Degree
  • Registration with the NZ Social Workers Registration Board with current Annual Practising Certificate
