About the Role

The Specialist Community Health Service is growing, and we are seeking motivated, passionate nurses to join our Rheumatic Fever Clinical Nurse Specialist (CNS) team.
We are looking for a dynamic, self-directed, and energetic clinician who will thrive in a diverse, fast-paced community environment. If you are confident working with tamariki, rangatahi, and adults across the lifespan, this role could be an excellent fit for you.
As a Rheumatic Fever CNS, you will provide advanced clinical expertise and leadership for people living with Rheumatic Fever and Rheumatic Heart Disease, delivering care that is culturally responsive, evidence-based, and person- and whānau-centred.
Key Responsibilities
- Clinical Leadership - Provide advanced nursing expertise, leadership, and support to community-based nursing teams, and other Te Whatu Ora services caring for patients with Rheumatic Fever.
- Collaborative Care- Work in partnership with multidisciplinary teams across paediatric and adult services to deliver holistic integrated, whānau-centred care.
- Acuity Management- Act as a primary clinical resource supporting patients and whānau diagnosed with Rheumatic Fever.
- Service and Strategic Delivery- Contribute to the ongoing development and delivery of the Rheumatic Fever service across Te Whatu Ora Counties Manukau services, ensuring excellence in nursing practice, and supporting excellence in clinical practice that is sustainable, culturally responsive, and aligned with best practice.
About You
You will have:
- Registration with the New Zealand Nursing Council and a current Annual Practising Certificate
- A minimum of 5 years’ post-registration nursing experience
- A Postgraduate Diploma in Nursing
- Preferred Clinical experience working with patients with Rheumatic Fever and/or Rheumatic Heart Disease
- A current Proficient/Expert or Senior Nurse portfolio
- Advanced clinical assessment skills
- Excellent communication and interpersonal skills
- Demonstrated innovative and evidence-based nursing practice
- A strong commitment to holistic, person- and whānau-centred care across the lifespan
- A solution-focused, collaborative approach and the ability to work effectively as part of a broad team of professionals
- A clean full New Zealand driver’s licence
