Are you an experienced mental health nurse looking for a senior clinical role with the opportunity to lead integration across primary and secondary services? If so, Health New Zealand Te Whatu Ora – Counties Manukau invites you to join our dynamic Mental Health Services for Older People (MHSOP) Community team as a Clinical Nurse Specialist.
In this advanced nursing position, you’ll play a pivotal role in strengthening the relationship between MHSOP and the primary care sector. You’ll work to improve access and care coordination for older people with mental health needs, bridging the gap between primary, community, and specialist services. This includes developing effective communication pathways, improving clinical integration, and ensuring recovery-focused, whaanau-centred care.
You’ll be part of a virtual team of Clinical Nurse Specialists reporting to the Nurse Lead for Integrated Care and Team Manager, working collaboratively to deliver equitable, high-quality care across the non-urgent mental health pathway.
Key Responsibilities:
- Build and maintain strong partnerships with GPs, practice nurses, NGOs, locality groups, and PHOs.
- Undertake comprehensive mental health assessments and lead appropriate care planning and referrals.
- Provide expert consultation and education to primary care teams to enhance mental health literacy.
- Facilitate wellness planning and health promotion with older adults and their whaanau.
- Lead or contribute to clinical quality initiatives, projects, and integration of services.
- Participate in cross-sector forums, strategy development, and service improvement activities.
- Serve as a key contact for non-urgent referrals, ensuring timely triage and coordinated care.
Salaries are based on the PSA Mental Health & Public Health Nursing Collective Agreement, Senior Nurse Scale.

Essential requirements
- NZ Registered Mental Health Nurse
- Current Annual Practicing Certificate
- Minimum of 5 years post registration experience in Mental Health with some of this being within a community mental health team
- Relevant post graduate qualification completed or near completion
- Full, NZ driver’s licence with no limitations
