About the role:
We are looking for experienced nurses and patient champions to work in the Community Health Service within a centralised Hospital in the Home team. This Clinical Nurse Specialist role provides acute community based nursing for patients to support admission avoidance and early discharge pathways. The Clinical Nurse Specialist works as a member of an interdisciplinary Hospital in the Home team which includes Senior Medical Officers, Nurse Practitioners, Clinical Nurse Specialists, Registered Nurses, Maori team, Allied Health and Health Care Assistants staff working across the Counties Manukau Health district.
The Hospital in the Home team will provide a mixture of remote monitoring and face to face patient assessments/intervention with rostered shifts between 8am-7pm, Monday - Sunday. The CNS will:
- support adapting the Te Whare Tapa Wha framework to patients and working alongside their whanau in the community.
- provide expert, evidence-based care to optimise patient outcomes and ensure the services delivers high quality care.
- be effective role models demonstrating high levels of nursing competence and knowledge sharing,
- provide leadership, guidance and support within the team
initiate and develop innovative approaches to problem solving while utilising skills and input from other disciplines and team members to co-ordinate patient care. be empowered to make decisions or recommendations relating to patient management, while demonstrating a high level of communication and professionalism.
Essential requirements:
- Registered with the NZ Nursing Council
- Completed post grad diploma studies which must include advance clinical assessment paper and pharmacology.
- A minimum of 5 years recent clinical experience preferably in primary or community care
- Knowledge and experience in long-term conditions care coordination and case management
- Full Drivers Licence
