About the Role

The responsibility of a Clinical Nurse Coordinator within the Community Nursing Service is to manage the workflow into the service through the monitoring of referrals for services that are received from the primary, secondary and tertiary services as well as NGO’s. To complete the workload management tool daily, which assists to ensure an equitable distribution of patients, that align with the skills of the Registered Nurses rostered on duty. Support the Registered Nurses working in the community environment with the development of clinical skills and competencies required within the service, provide advice for trouble shooting clinical concerns that may arise for the Registered Nurses, and at times provide clinical care in the home or clinic settings. The role requires you to foster and maintain relationships and links across the health sectors (Medical and Nursing teams, Community Health providers, patients and their whanau) The work is varied, rewarding and interesting. If you have not previously worked in the community environment, this a wonderful opportunity to support our staff to provide quality care in the patients’ home and/or clinic environment.

About the Team/Service/ Location

The wider Community Nursing Service teams consist of the District Nursing teams in Napier and Hastings, the Continence, Ostomy, Better Breathing Hawkes Bay (providing respiratory and heart failure rehabilitation and Community Oxygen Therapy) and the Wound and Community Clinical Nurse Specialist teams, as well as Clinical Nurse Coordinators, Health Care Assistants along with administration team support. The services are based at Hawkes Bay Fallen Soldiers Memorial Hospital and Napier Health Centre, requiring you to work from the either base. Referrals are received from the primary, secondary and tertiary health sectors as well as NGO’s.

About you

You will need the following key attributes and skills:

  • Previous experience working in a community setting
  • 3 -5 years post graduate experience
  • Recent experience within a clinical setting
  • Full unendorsed drivers’ licence
  • Computer skills
  • Exceptional customer service skills
  • Reliable and trustworthy
  • Previous experience working within a team
  • Ability to work autonomously
  • Experience in a leadership role
