We are seeking an experienced and dynamic nursing leader to join our Adult Community Services leadership team as Charge Nurse Manager – Intermediate Care Services.

This is an exciting opportunity to lead a highly skilled multidisciplinary team delivering innovative models of care that support people to receive treatment, rehabilitation and recovery in the place they call home.

Te whiwhinga mahi | The Opportunity

As the Charge Nurse Manager, you will provide operational, professional and strategic leadership across our Intermediate Care Services, comprising:

Hospital in the Home

Providing acute-level care in people's homes as an alternative to hospital admission, supporting individuals with a range of medical conditions to recover safely in their own environment.

Rapid Community Access Team (RCAT)

Delivering intensive short-term nursing and multidisciplinary interventions for people experiencing acute health deterioration, helping prevent avoidable hospital admissions and supporting timely discharge from hospital.

Interim Care

Providing short-term, restorative support for people transitioning from hospital back into the community, with a focus on recovery, rehabilitation and maximising independence.

These services operate across hospital and community settings and are central to our vision of delivering integrated care closer to home.

As the frontline leader, you will be responsible for leading a multidisciplinary workforce of nursing, allied health and support staff, ensuring safe, high-quality and equitable care while driving service innovation and continuous improvement.

You will work collaboratively with clinical leaders, operational managers, primary care providers, community partners and hospital services to achieve excellent outcomes for patients and whānau.

Key Responsibilities

  • Provide visible clinical and operational leadership across Intermediate Care Services.
  • Lead, coach and develop a multidisciplinary workforce.
  • Foster a culture of excellence, innovation, quality improvement and person-centred care.
  • Ensure safe, effective and equitable service delivery across community and hospital interfaces.
  • Drive service development and transformation initiatives that support care closer to home.
  • Support achievement of service performance indicators, quality standards and financial sustainability.
  • Build strong relationships with internal and external stakeholders to enhance integrated care pathways.
  • Champion Māori health gain and equitable outcomes for Māori, Pacific peoples and other priority populations.

Nga Pūkenga Motuhake | Important Skills

We are looking for a leader who is passionate about community-based healthcare and thrives in a complex and evolving environment.

You will have:

  • Current New Zealand Nursing Council registration and Annual Practising Certificate.
  • Postgraduate qualification in nursing or health leadership, or be working towards this.
  • Significant post-registration clinical experience, including leadership experience.
  • Proven ability to lead and inspire multidisciplinary teams.
  • Experience managing change and service improvement initiatives.
  • Strong understanding of quality improvement, risk management and clinical governance.
  • Excellent communication, relationship management and influencing skills.
  • Experience working across hospital and community settings would be highly advantageous.
  • A demonstrated commitment to Te Tiriti o Waitangi and achieving equitable health outcomes.
