About the Role

Join the Intake Team within Mental Health Services for Older People and play a key role in supporting tangata whaiora and whānau through assessment, triage, crisis response, and care coordination. This role focuses on delivering high quality mental health support that promotes independence, wellbeing, and safe community living for older people across the Bay of Plenty.

About the Team/Service/Location

MHSOP Community Team works closely with referrers, health partners, primary care, and community agencies to provide responsive and recovery focused mental health care for older people. Grounded in solid values of compassion, all-one-team, and excellence, the service is committed to partnership with whānau and community to achieve positive outcomes. Based in Tauranga, the team works within a supportive multidisciplinary environment focused on collaboration, continuity of care, and improving quality of life for tangata whaiora.

About You

You will be an experienced Social Worker with a strong background in mental health and a genuine commitment to supporting older people and their whānau. You bring excellent assessment and relationship building skills, work well within multidisciplinary settings, and remain calm and focused within complex clinical situations. Your practice reflects a sound understanding of mental health legislation, recovery focused care, and the importance of culturally responsive and person-centred support.

What you will bring:

  • Registration with the Social Work Registration Board and a current Annual Practicing Certificate
  • Minimum of three years postgraduate experience within mental health services
  • Experience working within older persons mental health (preferred)
  • Strong understand of mental health conditions, dementia, and delirium care
  • Knowledge of the Mental Health Act, PPPR Act, and capacity assessment processes
  • Experience in triage, assessment, crisis response, and care coordination
  • Strong communication and relationship management skills with tangata whaiora, whānau, and community agencies
  • Commitment to collaborative, culturally responsive, and recovery focused practice
