About the Role
As a NASC Triage Clinician, you will be the clinical point of entry for our service. You will utilise your professional expertise to assess and prioritise incoming referrals for adults with long-term disabilities or chronic health conditions, ensuring their needs are met within a locality-based framework.
Key Responsibilities:
- Clinical Prioritisation: Review and action incoming referrals, ensuring those with high clinical risk or urgency are identified and addressed promptly.
- Clinical Decision-Making: Apply your diagnostic skills to determine eligibility and identify the most appropriate clinical pathway for patients to remain safely at home.
- Multidisciplinary Liaison: Act as a key clinical contact for GPs, hospital teams, and community providers to streamline support services.
- Risk Assessment: Identify complex cases and "red flag" situations that require specialist input or immediate intervention.

About you
We welcome applications from Nursing or Allied Health Clinicians with practice experience and who meet the following criteria:
- NZ registered health professional with current annual practising certificate
- Full NZ driver’s licence
- Open to new challenges and experiences
- At least 3 years of experience within a NASC team is essential
- Commitment to the development and implementation of quality improvement initiatives
