Registered Nurse – Haemodialysis Case Manager

Location: Home Dialysis Unit, Greenlane Clinical Centre

An exciting opportunity has become available for experienced Registered Nurses to join our dedicated team of haemodialysis trainers and case managers at the Greenlane Haemodialysis Centre.

In this rewarding role, you’ll deliver contemporary renal care through a case management model, ensuring patients receive coordinated, evidence-based care that is patient-centred, culturally sensitive, and holistic. You'll be instrumental in providing continuity of care to patients with complex, chronic health needs.

In addition, you will provide skilled nursing assessments to assist members of the multi-disciplinary team (MDT) in implementing and adjusting patient care appropriately including pre-dialysis assessments.
