About the Role
The Pou Tohu position provides referral coordination & triage for primary and out of area referrals to our North Adult mental health team.
It also provides Service Facilitation/navigation across the team and the wider mental health and addictions sector for whai ora to access community-based support services and residential services.
As the Pou Tohu you will be instrumental in ensuring whai ora and whanau have a positive first experience engaging with mental health services with an emphasis on whakawhanaungatanga and manaakitanga in all interactions, laying the foundation for positive engagement and support.
Service facilitation and development of a support plan is also vital to enable access to individualised support services based on assessed needs in partnership with whai ora, their whanau and the wider multi-disciplinary team, enabling whai ora to live well in the community.

About you
· To be successful in this role you must have a passion for the mental health field with a priority for improving the quality of lives of people experiencing mental health distress or living with a significant mental illness.
· Our ideal candidates will have advance mental health assessment skills, a broad range of experience in mental health services across service settings and a good knowledge base of services available in Taranaki both within and focused on specialist mental health providers but also community based and primary services to sign post whanau to.
· We are looking for health practitioners with current APCs and scope of practice to enable working in mental health services as well as a commitment and enthusiasm to learn new skills and systems.
