About the Role
As a physiotherapist working in the Interim Care Service (ICS) you will work primarily with patients who have presented with acute injuries or conditions that require short-term admission to an ARC (Aged Residential Care) facility for a period of recovery and rehabilitation, and for whom discharge to their own home is not possible in the short term.
You will work closely with the inter-disciplinary team (physiotherapists, occupational therapists, social worker and allied health assistants) and ICS charge nurse manager to provide ongoing assessment, rehabilitation and discharge planning. You will help coordinate and develop strong links between the acute and community teams and build relationships with the ARC facilities to ensure a high quality of service is provided to patients and whānau.
This contract also includes supporting the inpatient allied health rehabilitation team for a few hours a week (4 hours), who provide rehabilitation to older adults at North Shore Hospital. The inpatient work covers an array of conditions including patients with orthopaedic, neurological and general medical conditions. You will join an established inter-disciplinary team providing quality intervention, rehabilitation and often complex discharge planning through a patient-centred approach. The AT&R team has strong links with the ICS team, our Early Discharge and Rehabilitation Service (EDARS), community, and outpatient services.
About the Team/Service/ Location
The interim care service is led by a charge nurse manager and has a team of two physiotherapists, two occupational therapists, a social worker and two allied health assistants. They work closely, providing inter-disciplinary rehabilitation, supporting patients to get home as quickly as possible from their temporary stay in an ARC. You will be based at North Shore Hospital and will travel to the ARC facilities (which are located within our Waitematā catchment area on the North Shore and in West Auckland) to provide rehabilitation. Staff are provided with a mobile phone and laptop so mobile working is as efficient and effective as possible.
About you
Waitematā is looking for an experienced Physiotherapist who is passionate about rehabilitation and who is excited to work across two teams within the inpatient and community sectors. This is a great opportunity to be a part of our Interim Care Service (ICS) and our Assessment, Treatment and Rehabilitation (AT&R) allied team at North Shore Hospital.
- New Zealand registered Physiotherapist
- 2+ years of clinical experience: recent experience within an orthopaedic and /or rehabilitation hospital setting and/or experience working with older frail adults is preferable
- Ability to lead, support and mentor/supervise junior staff and /or students
- Work well across inpatient and community teams to ensure a patient centred and evidenced based approach to their practice.
- Strong clinical assessment skills, great communication and organisational skills
- Experience with student supervision
- Drivers licence (restricted or full)
