Nurse Practitioner, Hospital in the Home
The hospital-in-the-home service has a new and exciting opportunity for Nurse Practitioners / Nurse practitioner intern, to join our energetic and friendly team. You will receive SMO collegial support, and work with skilled experienced nurses and the administration team. The expansion of the service is supported by a Regional governance platform.
Working within an acute hospital in the home environment you will need to be able to work autonomously and as part of a team.
Part of your role will be conducting physical assessments, diagnosing, and treating patients with acute / sub-acute conditions, and prescribing medications to support to our patients at risk of deterioration. Providing one-on-one advice to patients means that you will need to be friendly, have excellent communication and interpersonal skills, compassionate, and proactive in the management of conditions. Other facets of the role will include telehealth, education, and support of staff along with an active role in service and policy development.
The right candidate will ideally have skills in contemporary Acute and Emergency, General Medicine, General Practice, Acute clinic, or Urgent care experience at an advanced practice level however, we are keen to hear from NPs with other secondary care or community-based experience NP’s with transferrable skills. We also welcome interest from suitably qualified staff that may be interested in an internship.
