We are seeking an experienced Nurse Practitioner to join our innovative Hospital in the Home (HiTH) service. This is an exciting opportunity to deliver acute hospital-level care in patients’ homes, while contributing to the growth of virtual and community-based healthcare.

About the Role
The Hospital in the Home (HiTH) service provides acute inpatient-equivalent care in the community, using skilled clinicians, advanced technology, and robust clinical governance to deliver safe, high-quality care.
As a Nurse Practitioner, you will:
- Provide advanced clinical assessment, diagnosis, and treatment for patients receiving hospital-level care at home
- Work collaboratively with medical staff and a multidisciplinary team
- Support patients and their whānau to maintain autonomy and wellbeing while receiving acute care in their home environment
- Contribute to service development, innovation, and clinical leadership within the HiTH model

About You:
To be successful in this role, you will have:
- RGN; RGON; RPN; RCpN with or without BN or BHSC (Nursing)
- Registration with Nursing Council of New Zealand (NCNZ)
- Evidence of competence to practice within the Nurse Practitioner scope of practice
- A current Annual Practicing Certificate (APC)
- A minimum of 4 years of experience in a specific area of practice
- A commitment to achieving equitable outcomes for Māori
- Completion of an approved clinical Master’s degree
- A personal commitment to on-going learning and development
- Research skills including data analysis and critical appraisal skills and clinical audit.
- Knowledge of quality improvement processes and principles
- Excellent presentation and communication and skills

The Hospital in the Home service delivers acute hospital care outside the traditional hospital setting.
You will join a supportive, experienced team that values collaboration, innovation, and continuous improvement. We provide a comprehensive orientation and ongoing professional support to ensure you feel confident and well supported in your role.
