Needs Assessment and Service Coordination - Adult Mental Health Services

  • Full-time role (40 hours per week)
  • Based in Paramount Drive, Henderson and North Shore.

This is your opportunity to be part of an evolving person centred, Mental Health NASC team, who have a focus on improving the quality of the service provision and the outcomes from engagement. 

About the Role

In this role, you will undertake unprejudiced comprehensive assessments to identify both the person’s strengths and areas for development across all aspects of life. You will work closely with the person and whanau, to identify goals and plan for support. You will work with all relevant stakeholders, including clinical services, other state agencies and the NGO sector.

About You

You will be an experienced registered health professional with a valid APC, who has had a minimum of 5-years’ experience, working in mental health. You will be self-directed and motivated and be able to work across a number of service providers with collaborative practice skills to ensure the best assessment, matching of need and supports for recovery. You will be a star in using basic Microsoft packages, including Outlook, Word and Excel and have experience in diary management. Knowledge of electronic clinical record systems would be an advantage. 

This is a full time, Monday to Friday role based on the Northshore but working across the Waitemata district. 

Ideally, you will be NZ registered Social Worker/OT with a current APC. If you are from overseas and eligible to apply registration and APC, we are way more than happy to hear from you

A full and clean NZ driver licence is essential in this community based role.
