The Role:
We are seeking an experienced and passionate Clinical Nurse Specialist (CNS) to join our dynamic multidisciplinary renal team. In this specialised leadership position, you will play a vital role in coordinating and delivering expert clinical care and strategic leadership in the management of vascular and peritoneal dialysis access for patients across the continuum of care.
This is a pivotal role that ensures patients receive optimal access-related care through a case management model, working in close collaboration with patients, whaanau, and healthcare professionals both within HNZ Counties Manukau and across the region.
Key Responsibilities:
- Provide clinical expertise in the management of complex renal patients requiring access surgery and ongoing monitoring.
- Coordinate all pre-admission, consent, and surgical planning for patients requiring vascular or peritoneal access.
- Lead access care planning, triage referrals, and support high-risk patient monitoring using electronic systems.
- Promote innovative nursing practices based on best practice and current research.
- Actively participate in quality improvement, audit, and research projects relevant to renal access.
- Deliver education to patients, staff, and the wider multidisciplinary team.
- Act as a mentor and expert clinical resource for nursing staff and promote continuous learning and development.
- Collaborate in developing guidelines, standards, and strategic planning at local and national levels.

About You:
- NZ Registered Nurse with a current Annual Practicing Certificate
- Minimum 3-5 years renal nursing experience in NZ (post registration)
- Current PDRP demonstrating senior nurse attributes
- Relevant post graduate qualification completed or near completion
