About the Role

You will provide high-level, specialist clinical care to patients across the community and palliative care settings. This role involves comprehensive patient assessments, development of care plans, prescribing of ostomy consumables, and delivery of both formal and informal education to patients, whānau, and colleagues.

You will use your expert knowledge in stoma management to support complex clinical decision-making, contribute to quality improvement initiatives, and promote best practice care. Working collaboratively within an interdisciplinary team, you will liaise closely with the District Nursing Service and other community services, playing a key role in delivering coordinated and patient-centred care.

About the Team/Service/ Location

This role is based at Waitakere Hospital. The Community Ostomy Service sits within Community and Palliative Care at Waitematā and is made up of a supportive, experienced team of Nurse Specialists working across community sites. You will be part of a cohesive service that values collaboration, professional growth, and high-quality patient outcomes.

About you

The ideal candidate will be a Nurse Specialist in Ostomy or a Level 4 Registered Nurse with a current portfolio, and a minimum of three years' district nursing experience including contemporary current practice in the management of ostomy care.

As you will be relating to patients and staff at all levels, you will need to have excellent communication and problem-solving skills. You will be able to quickly establish relationships across multiple teams displaying Waitemata’s organisational values.

In addition, you are working towards or have completed a post-graduate diploma relevant to the role. Further support will be provided to further develop advanced practice skills.
