Clinical Nurse Specialist - Frailty Admission Avoidance Service

- Permanent – Part time, 32 hours per fortnight, or 0.4 FTE Monday - Friday

About the Role
The role of the CNS is to respond immediately to referrals to navigating solutions/pathways for patients in the community who have presented with complex social or health issues which do not require hospitalisation if these are addressed. The CNS holds strong links with both START and Disability Support Link, and case manages a cohort of patients to provide ongoing support.
The salary for this position will be based on experience and in line with the NZNO Te Whatu Ora – Health New Zealand Nursing and Midwifery collective agreement. Additional benefits include kiwisaver contributions (if eligible), shift allowances, professional development and wellness support.

About the Service
The Frailty Admission Avoidance Service is a small team of Clinical Nurse Specialists and a Nurse Practitioner who work directly with GP’s and other community health care providers to provide early interventions to vulnerable older people living in the community, with the aim of preventing emergency department presentations and hospital admissions.

Thames Hospital is a modern 50-bed secondary care facility offering a comprehensive range of services, including an emergency department, inpatient care, day surgery, an infusion unit, rehabilitation services, birthing unit and low-acuity mental health care. Our hospital boasts 24-hour laboratory and radiology services, including advanced CT imaging. As an accredited training hub, we support 5th and 6th-year medical students from Auckland University, PGY2 House Officers, and registrars in Rural Hospital Medicine and General and Acute Care Medicine. About you
To be considered for this role you will be:
- Registered Nurse with a current New Zealand practicing certificate
- Minimum of 5 years’ experience, preferably in the community and working with older people
- Advanced clinical and assessment skills
- Relevant post graduate education
- Ability to work independently and as part of an interdisciplinary team
- Ability to develop collegial relationships with all stakeholders
- Full New Zealand drivers licence
