About the Role
Be part of the innovative START (Supported Transfer and Accelerated Rehabilitation Team), a service that supports older people to regain their independence after an acute episode or injury. As a Rehabilitation Assistant or Healthcare Assistant, you will work under the delegation of registered health professionals to deliver individualised care and therapy in clients’ homes. This role is pivotal in enabling patients to recover faster, improve their functional resilience, and seamlessly reintegrate into their primary care routines.

The START team is a multidisciplinary group of dedicated professionals committed to enhancing patient outcomes by providing rapid and effective support to older people transitioning from hospital to home. Situated in the beautiful Bay of Plenty, this role offers the chance to work in a region known for its stunning beaches, lush landscapes, and a community-focused lifestyle.

About You
You will be a compassionate and motivated Rehabilitation Assistant or Health Care Assistant who is passionate about helping older people achieve their goals and maintain their independence. You enjoy working in community settings, building meaningful relationships with patients and whānau, and contributing to positive health outcomes. You are adaptable, organised, and confident working both independently and as part of a multidisciplinary team. Your approach is patient-centred, and you take pride in supporting people to maximise their recovery and remain safely in their own homes.
What you will bring:
- Experience as an Allied Health Assistant, Rehabilitation Assistant, Health Care Assistant, or similar support role
- Strong communication and relationship building skills with patients, whānau, and health professionals
- Ability to work collaboratively within a multidisciplinary team and follow delegated rehabilitation Programmes
- Passion for supporting independence, recovery, and wellbeing in community settings
- Excellent organisational and time management skills
- Commitment to culturally responsive, equitable, and person-centred care
- Full New Zealand driver’s licence
