We have an exciting opportunity for an experienced pharmacist to join our interdisciplinary team who has experience working in the hospital and/or community setting. This role has been established as part of the community restructuring. In this influential role, you will have the opportunity to work with a diverse range of clinical presentations, solving real-life challenges and contributing to changing lives for the better for many in our community. You will work closely with the SMO, Nurse Practitioner's, complex case nurse's, acute services medical teams and local community GP's to support admission avoidance and the supported transition of patients from hospital acute services into their homes.

Your main responsibilities include:
• Providing clinical pharmacy services to our community patients including medicines reconciliation, clinical reviews, optimising therapy, therapeutic drug monitoring, medication adherence assessment, medication counselling and discharge facilitation and attending MDTs
• Ensuring evidence based, safe and effective use of medicines and patient-centred care
• Contributing towards/leading quality improvement and medication safety initiatives
• Continuing to grow your clinical expertise and contributing to education and training of your colleagues and other staff
Successful applicants must meet the following criteria:
• New Zealand Registered Pharmacist with current APC or eligibility to obtain one
• At least 5 years relevant experience
• Motivated and positive attitude
• Excellent communication and interpersonal skills
• Prior hospital and /or experience with established clinical pharmacy competencies and technical ability
• Relevant postgraduate qualifications in clinical pharmacy (preferred)
