You will help coordinate and develop strong links between the acute and community teams and build relationships with the ARC facilities to ensure a high quality of service is provided to patients and whānau. 2025 will bring further growth of this service into the community rehabilitation space, providing service development and quality improvement opportunities.
The AT&R wards at North Shore Hospital provide rehabilitation to older adults. The work covers an array of conditions including patients with orthopaedic, neurological and general medical conditions. You will join an established inter-disciplinary team providing quality intervention, rehabilitation and often complex discharge planning through a patient-centred approach.
