About the role:
Would you like to contribute to the future of health care in the South Auckland area?
We are looking for a Nurse Practitioner to provide clinical expertise and leadership to the Hospital in Home and the Specialist Community Health Teams.
In this role, you will practice at the top of your scope, providing clinical leadership and direct patient care through home visitation, virtual consultations, and clinic reviews in collaboration with our multidisciplinary teams. You will support and mentor community nursing staff, co-design and implement evidence-based care pathways, and help deliver timely, hospital-level care in the community that supports recovery, independence, and whaanau wellbeing.
This is an exciting opportunity to work alongside a growing Nurse Practitioner team and supportive Senior Medical Officers as we continue to strengthen our integrated model of care that brings hospital-level services into the home. You will play a key role in advancing innovative service models, including remote monitoring using wearable technology, virtual ward integration, community-based IV therapy, and pathway development for complex and multimorbid conditions, while embedding equity-focused and culturally responsive practice across all services.
This position offers flexible 8-12 hour shifts working across the 7 days. Weekend shifts are expected to be 1 in 5 weeks.
Essential requirements:
- Registered with the NZ Nursing Council
- Current APC with the scope to work as a Nurse Practitioner
- A minimum of 5 years recent clinical experience in a relevant area
- Completed a clinical Master’s degree
