We are seeking an experienced, client-focussed health professional to join our dynamic needs assessment team. We have a permanent 1.0 FTE role available, working within our community team. This role will be flexible and involve assessing people in their homes within the community. We have NASC teams and the locality bases are in Botany, Otara, Mangere, Papakura, Pukekohe and the Middlemore site.
This essential role, forms part of the interdisciplinary team and involves assessing the needs of adults experiencing health issues. You will be working with various team members in partnership with clients and family/ whaanau. You will be carrying out specialised assessments using the internationally standardised InterRAI assessment tool and putting care plans in place, to ensure that clients are supported to remain or return to their own homes, residential care facilities or other community living settings.
In these roles you will have the support of the interdisciplinary team and supervision provided by senior colleagues, and will receive a comprehensive orientation.
Essential:
- NZ Registered Health Professional with a current Annual Practising Certificate and NZ Professional body registration such as a Registered Nurse, Occupational Therapist, Social Worker, Physiotherapist, Pharmacist or Dietitian.
- Full NZ drivers Licence
- Comprehensive clinical assessment skills
- Excellent verbal, written communication and computer skills
- 3 years’ experience working in a multi-professional, inter-disciplinary health or disability-related environment
- Be able to meet the demands of a fast-paced health environment
- Flexible working style and able to work across different settings
- Client and Family/Whaanau focussed
Preferred:
- InterRAI assessment trained
- Experience in working with adults of all ages, including older adults in a hospital or community setting
- Experience supporting clients with both Physical and Mental Health issues.
