About the role:
Are you passionate about helping people improve their health and wellbeing? We are looking for an enthusiastic Community Health Worker to join our Sleep team at Manukau Health Park.
In this role, you will support patients and their whaanau with sleep and respiratory needs, working alongside our nurses, physiologists, doctors and wider health team. You’ll provide education, equipment support, and follow-up care for people using therapies such as CPAP for obstructive sleep apnoea (OSA). You’ll also be a vital cultural link for patients and their whaanau, helping to connect them with the services and resources they need.
What you’ll do:
- Provide education on some diagnostic tests for OSA.
- Provide education and support for patients in the hospital, clinic, and home.
- Assist with CPAP set-up, oxygen equipment management, and troubleshooting.
- Run clinics and help patients and whaanau manage equipment and treatment plans.
- Maintain patient databases and help with service coordination.
- Sleep equipment stock management and delivery.
- Work closely with Maaori and Pacific communities to ensure care is accessible and culturally responsive.
About you:
- Strong communication and interpersonal skills.
- Organised, reliable, and able to prioritise tasks effectively.
- Experience working with people from diverse cultural backgrounds.
- Practical experience in health, community, or customer service roles (advantageous).
- Comfortable using computers and common applications (Excel, Outlook, databases, MS Teams).
- Full NZ driver’s licence.
- Knowledge of Te Reo Maaori and/or a Pacific language (desirable).
- Passionate about making a difference for patients, whaanau, and the wider community.
