Clinical Nurse Specialist (Full Time) –Rapua Te Ao Waiora, Adult Community Mental Health

We are seeking an Experienced Mental Health Nurse to join Rapua Te Ao Waiora, a vibrant and culturally diverse community of South Auckland

About the role:

Are you an experienced mental health nurse ready to take the next step in your career? We're seeking a dynamic and passionate Clinical Nurse Specialist (CNS) – Primary Care Integrated Pathway to lead and champion integrated, recovery-focused care across the primary and secondary health sectors.

In this pivotal role, you’ll work closely with primary care providers, locality teams, and other community groups to enhance coordination, support recovery, and ensure excellent physical and mental health outcomes for our service users. You will be part of a virtual team of CNS’s working across our four Community Mental Health Centres (CMHCs).

Key Responsibilities:

  • Clinical Leadership: Deliver advanced clinical assessments and interventions, with a focus on the non-urgent pathway. Support the development of integrated care pathways and discharge planning. Lead and support the nurses in your team.
  • Collaboration & Integration: Build and maintain strong relationships between CMHCs, primary care, and community providers to enable seamless care and support service users to navigate complex systems.
  • Health Promotion & Wellness: Drive initiatives that support improved physical health and wellness for mental health service users. Promote self-management and health literacy.
  • Care Coordination: Facilitate person- and whaanau-centred care planning, ensuring services are aligned with each individual's goals.
  • Education & Mentorship: Provide expert consultation and support to primary care providers and act as a mentor and educator for nursing and clinical colleagues.
  • Quality & Innovation: Lead and participate in continuous quality improvement initiatives, research, clinical audits, and policy development.

Essential requirements 

  • NZ Registered Mental Health Nurse
  • Current Annual Practicing Certificate
  • Minimum of 5 years post registration experience in Mental Health with some of this being within a community mental health team
  • Functioning at RN Proficient Level or above
  • Relevant post graduate qualification completed or near completion
  • Full, NZ driver’s licence with no limitations
