About the role:
As a Clinical Nurse Specialist - Patient Flow you will provide clinical leadership and oversight of surgical patient flow across Te Whatu Ora (Health NZ) Counties Manukau, with a focus on coordinating safe, efficient admissions, discharges, and transitions of care. Working closely with multidisciplinary teams, you will ensure patients and their whaanau experience seamless, high-quality care, while leading well-coordinated discharge planning from the point of admission. You will also support and educate staff to build capability in delivering effective, patient-centred discharge processes.
Key Responsibilities:
- Deliver specialist nursing expertise to patients and provide holistic support to their whaanau across the continuum of care
- Collaborate closely with multidisciplinary teams to ensure patients experience integrated, high-quality care within the hospital setting
- Provide strong nursing leadership to coordinate patient treatment, education, and discharge planning in alignment with specialty needs
- Support and educate staff to enhance their capability in discharge planning, ensuring preparedness begins at the point of admission
- Will be part of a team supporting the provision of care across a 7 day period, morning and afternoon shifts.
Essential Requirements:
- NZ Registered Nurse with current APC
- Completed a post graduate diploma relevant to specialty
- A minimum of 5 years (post registration) surgical nursing experience
- Functioning at Proficient or above on the PDRP framework
Desired:
- Completed Post Graduate Masters
- Previous senior nursing experience
- Previous nursing leadership or management experience
