About the role: 

Calling all Clinical Nurse Specialists/Senior RNs ready to take the next step –The Community Liaison team are looking for you!!

The Community Liaison team are an hospital-based team responsible for promoting effective and efficient acute flow of patients transitioning between acute hospital-based care and the Community Health Service. You will be part of the team who proactively identifies in-patients that are appropriate for transfer to community-based care. You will work with a range of teams and clinicians including ED and associated acute areas, as well as medical, surgical, and speciality wards to support safe, timely discharges and seamless transitions of care.

You will be part of the FERN team, a dynamic service focused on supporting patients with extended hospital stays. As a valued member of this interdisciplinary team, you will help coordinate care, arrange services, and facilitate timely discharge planning for these patients, ensuring they receive the right care in the right place at the right time.

 

This role will require:

  • Current APC
  • Postgraduate Diploma in Nursing or working towards
  • Acute and Community nursing experience
  • Excellent communication and interpersonal skills
  • Current Proficient/Expert or Senior Nurse portfolio
  • Innovative and creative clinical practice that is validated with evidence based best practice
  • Liaising with in-patients, Emergency Care, Manukau Health Park, Ambulatory and Specialist Community Health Service staff and Non-Government Organisations (NGOs)
  • Collaboration with the Community Health Teams to ensure that care is appropriate and provided in a timely manner with appropriate resources available to safely transition patient care.
  • Passion for working in a person/whaanau centred way to provide excellent care
  • Solution focussed, team player with excellent communication skills.
  • Advanced assessment skills

The successful applicant will have proven communication skills including conflict resolution, leadership skills and experience in service development. Ongoing support and training will be provided to develop your leadership skills in clinical assessment, case reviews, performance reviews and complaints procedures.
